- 3 y/o Husky (Luke) died by hanging. Arm got stuck in kennel door and hanged there until he died. (Critical, WTF)
An approximately 3 year old male husky named "Luke Nikolas VII" died on 12/8/24. The licensee found the dog hanging by his arm in between the kennel door and front part of the kennel. It appears the dog most likely jumped up and pushed the door somewhat forward in the process. While doing that the dog got its arm stuck and was unable to free himself hanging there until he was found deceased. Housing facilities must be designed so they are structurally sound and protect the animals from injury. The licensee must ensure all his kennel doors don't allow for the dogs limbs to get stuck in the opening of the door between the gate the kennel wall. Correct starting on 8/5/25.
- No emergency plan
The licensee does not currently have a contingency plan in place. Not having a contingency plan doesn't ensure the facility will be prepared in the event of an emergency or disaster. The licensee must complete the contingency plan in accordance with the requirements outlined in the Animal Welfare Act.
- Vaccines not up to date
All six adult dogs are not current on their distemper or parvo shots. Not vaccinating all adult dogs against distemper and parvo doesn't ensure those dogs are adequately protected against those diseases. All adult dogs must be vaccinated against distemper and parvo and record must be kept documenting those vaccinations.

- Expired medication in use
There is an expired bottle of medication still in use in the kennel. A bottle of Safeguard (dewormer) expired 09/2025. The licensee reported he has used the medication since the expiration date. Expired medications are improper for usage as the efficacy, potency, and freshness cannot be confirmed. If the medications are no longer effective, normally preventable diseases may become a problem.
