Examination of APHIS form 7005, Record of Acquisition and Dogs and Cats on Hand, revealed that there is no official USDA identification number for any of the dogs. Licensee stated that none of the dogs had official USDA identification, but each dog is identified by an individual name and this is noted on APHIS form 7005. Official USDA identification is necessary to complete all required paperwork. Licensee must provide each dog that is four months of age or older with proper USDA identification using either approved ear tattoo, approved microchip or approved collar tag affixed to the dog's neck. These identification numbers must be added to APHIS form 7005 as well as, any other APHIS form that requires it. Correction date: March 2, 2015

An enclosure, housing two adult dogs, had wood particular board from the primary fencing. This wood board was excessively chewed and scratched. This wood surface cannot be properly cleaned or sanitized. As a result, the disease hazards can increase for the dogs. For the prevention of disease hazards to the animals, all surfaces must be maintained on a regular basis and kept in good repair for proper cleaning and sanitation. To be corrected by, 2-March-2017.

The dog doors for six enclosures, housing 10 adult dogs, had an accumulation of brown to black grime or debris on their surfaces. The door inserts and area around some of the dog doors had an accumulation of brown to black grime or debris. This indicates daily spot-cleaning is not taking place and could increase the disease potential for the animals. All surfaces must be spot-cleaned and disinfected regularly for the prevention of an accumulation of waste or grime. This will decrease the disease hazards for the dogs. One enclosure, housing one adult dog, had wire used to fasten the panel divider to the back wall. This wire had sharp protruding points. The points were protruding into the enclosures at approximately eye level of the dog in normal standing position. These points could cause injury to dog. All enclosures must be kept free of sharp protruding points that could injury the animals. To be corrected by, 21-February-2017.
