  • Weaned puppies identified by cage cards

    Kennel number 1 features 11 weaned puppies from 3 litters, all under 16 weeks of age, identified by cage cards rather than the methods prescribed by this section. Deviations in puppy identification may compromise tracking of individual puppies on the premises. The facility must identify weaned puppies less than 16 weeks of age by an official tag as described in section 2.51, an Administrator-approved tattoo marking, or an Administrator-accepted plastic-type collar displaying the information required by section 2.51. Correct by March 4, 2025.

  • Parasite prevention schedule not followed

    While the facility's written Program of Veterinary Care directs administration of ectoparasite prevention to adult dogs monthly between April and November, the facility has been administering it less frequently during these times. Deviation from the approved parasite prevention schedules may increase risk of parasitism, especially during months with potentially higher exposure risk. Facility parasite control practices must adhere to a program authorized by the Attending Veterinarian. Correct by March 14, 2025.

The scratching has deteriorated this section of the panel to where it can no longer be effectively cleaned and sanitized and could pose a potential hazard to the dog. This section of the primary enclosure must be kept in good repair for the safety of the dog. Correct by: Nov 30, 2025
