There are 59 dogs in the kennel which are not identified with an official tag, tattoo, or microchip. All adult dogs must be officially identified in the kennel with a tag, tattoo, or microchip to help identify and to keep track of animals. Owner plans to microchip all of his adult dogs on the premises. Licensee must ensure that dogs are offcially identified in the kennel. Correct by August 7, 2014

The same puppy number was listed on the Record of Disposition of Dogs and Cats (APHIS form 7006) for different puppies sold in 2015. Official tags or collars must be serially numbered so that animals sold have a unique number identifying each puppy. No individual dealer shall use any identification tag or collar number more than once within a 5-year period. Failure to properly identify each puppy with a unique number makes tracking of animals more difficult. Licensee shall ensure that all dogs and puppies sold are properly identified with a unique number. Correct by June 1, 2015

A three year old tan and white Pomeranian female (Wendy Mae, ID# 941000016857544) had an area of hair loss measuring 4 inches by 4 inches on tail base that has not fully grown back after being shaved before she whelped in February 2017. The underlying skin is dark grey where the hair failed to grow. The dog did not appear to be scratching at the area. There is no treatment record or diagnosis for this dog. According to the veterinary handbook, dogs that are not itching, but the hair does not grow back, should be examined by the attending veterinarian for hormonal or immune disorders. A number of issues can cause skin disorders in dogs. This dog needs to be seen by the attending veterinarian for an evaluation and diagnosis. Correct by Monday, July 10, 2017.

Housekeeping. There is dirt and debris along the east aisle in front of the whelping exercise runs near the office and along the barn wall. Premises must be kept clean and in good repair to reduce and control living areas for rodents and to protect the health of the dogs. Licensee must keep the aisle in front of the exercise runs and along the barn wall free of dirt and debris to prevent pest problems. Correct by December 10, 2022.

The majority of the APHIS 7006 forms did not list the method of transportation of the carrier, intermediate handler, or name of the person who transported the dog or puppy. Failure to record the method of transportation makes it difficult to know if the buyer, seller, or commercial shipper transported the puppy or puppies. Correct by recording the method of transportation on APHIS form 7006, including the name of the carrier, intermediate handler, or owner's name if a privately owned vehicle was used to transport the dog or puppy. Correct by October 5, 2023.

A veterinary examination was ordered on (1) dog under section 402 (a) based on the warden's observations of the condition of dogs during an inspection. Kennel is in compliance with section 402(d) because the veterinary examination was completed within the 72 hour timeframe.

Wardens viewed primary enclosures that were caked with dirt, and dust.

Wardens viewed a buildup of spiderwebs, dust, rust on outside doors, and grime on multiple surfaces throughout the main kennel building which encourages pests and contributes to poor sanitary conditions.

Wardens were left waiting approximately 10 minuets upon arrival until kennel owner allowed us to start the inspection. Kennel owner later admitted to entering the kennel and moving dogs before allowing wardens to enter.

Wardens viewed the presence of rust and chipping of wire coating in a few primary enclosures and throughout the main kennel building that could not be sanitized.
