- Outdated program of veterinary care
The Program of Veterinary Care that was presented to the inspector is out of date. It was last signed by the attending veterinarian on 02/07/2013. A written program of veterinary care is required when the attending veterinarian is employed on part time basis in order to ensure that animals are provided with adequate veterinary care at all times. The licensee must establish a current written program of veterinary care with the current attending veterinarian. Correction date: 9/05/14
- Broken wire flooring in fifth enclosure
In the fifth enclosure from the doorway, that houses at least two dogs, the wire flooring is broken and twisted upward towards the dogs. The sharp ends of the wire can cause an injury to the dogs. The broken wires need to be removed, replaced or altered in a manner so they no longer pose an injury risk to the dogs. Correction date: 9/10/14
- Chewed vinyl enclosures
The enclosures facing the doorway are made of vinyl and have been excessively chewed by the dogs. This damage allows dirt and food waste to penetrate the surface of the vinyl which can result in disease hazards towards the dogs. The vinyl needs to be replaced or altered in a manner that allows for proper cleaning and sanitization of the enclosure every two weeks. Correction date: 9/17/14
