- Housing over 85 degrees, no ventilation
Housing facilities for dogs had a temperature greater than 85 degrees and there was no auxiliary ventilation from fans, blowers and/or air conditioners present.
- Dispense records use PO Box, altered
Warden viewed records kept that did not comply with the requirement standards; specifically utilizing only a PO Box, not the physical address, of where a dog was dispensed. The kennel owner had also whited out the information on where a dog was dispensed because the person had returned it back to them a couple of weeks after the dog had left.

Warden viewed multiple areas in the kennel that had broken pieces of fencing that could pose injury to the dogs contained therein.

Warden viewed records for dogs that did not contain all dogs that were in the kennel. There was a littler of four (4) Yorkshire terrier puppies that were a couple of weeks out and not listed on records.
- Puppy transferred under 8 weeks
