One fiberglass wallboard has an estimated four-inch circular area of missing fiberglass. Three dogs are in contact with the affected wall board which is located in sheltered housing. Surfaces not maintained on a regular basis cannot be readily cleaned and sanitized. The open area between the fiberglass and wood wall could serve as a collection area for dirt, hair, fecal waste or other debris. The licensee must ensure that all surfaces must be maintained on a regular basis, can be readily cleaned and sanitized or be replaced when worn. Correct by 4/17/23.

The licensee currently does not have an adequate method of identification for 9 litters of weaned and unweaned puppies (29 puppies in total). The litters were born on various dates and the cage cards present do not contain individual identification, sex, breed, or description of the puppies. Proper identification of puppies is important for disease traceback and in cases of theft or loss. Correct by ensuring live puppies less than 16 weeks of age have proper identification, such as an official tag, tattoo or microchip, APHIS 7005 form, or a cage card. Correct by 24 December 2024

The facility is not following the program of veterinary care for vaccinations. The Program of Veterinary Care currently prescribes dogs will be vaccinated for DAPP and Leptospirosis annually. 3 adult dogs are not up to date on their Distemper, Parvo, or Leptospirosis vaccines: -Lela#4325 last administered on 3-7-2023 -Velvet#4333 last administered on 10-24-2023 -Flash#8520 last administered on 6-3-2023 1 adult dog did not have a record of a current Distemper or Parvo vaccine -Lexy#0930 Failure to follow the preventative care schedule for vaccinations in the Program of Veterinary Care does not ensure the dogs are receiving appropriate protection against contagious and deadly diseases such as distemper, parvo, leptospirosis, and rabies which can lead to illness and discomfort to the dogs. The facility must follow the vaccination schedule approved by the attending veterinarian in the Program of Veterinary Care to protect the dogs from deadly or contagious diseases. The facility is not maintaining adequate records of acquisitions. Cage cards are used in lieu of the 7005 forms for keeping acquisition records for litters of puppies. The facility has created a cage card for all the litters that have been born at the facility, but they do not contain all the required information. A litter of cava-poo puppies born on 13-July-2026 and a litter of cava-poo puppies born on 16-July-2026 have cage cards that are missing the individual identification, sex, and the color or distinctive markings of the puppies. A third litter of cava-poo puppies born on 25-July-2026 has a cage card that is missing the individual identification and the sex of the puppies. Accurate record keeping is essential so that APHIS inspectors can adequately identify the movement of dogs into and out of the facility. Each dealer shall make, keep, and maintain records for forms which fully and correctly disclose the information concerning each dog purchased or otherwise acquired, owned, held, or which is transported, sold, or otherwise disposed of by that dealer. The records shall include any offspring born to any animal while in his or her control. Correct by maintaining complete records for acquisition by 27-August-2026.
