  • Record of Disposition forms missing

    The facility has not been maintaining a Record of Disposition (APHIS form 7006) forms for dogs that have been routinely sent to another facility for whelping. Incomplete or inaccurate records do not allow for APHIS officials to properly track animal movements and to assess animal welfare. Each dealer shall use Record of Disposition of Dogs and Cats (APHIS Form 7006) to make, keep, and maintain the information required by paragraph (a)(1) of this section. Provided, that if a dealer or exhibitor who uses a computerized recordkeeping system believes that APHIS Form 7006 are unsuitable for him or her to make, keep, and maintain the information required by paragraph (a)(1) of this section, the dealer or exhibitor may request a variance from the requirement to use APHIS Form 7006. The licensee must ensure all required information is fully and correctly documented on an APHIS Form 7006 or by obtaining a variance. Correct by, 07-February-2025.

  • Rusted chain-link fence connections

    There are multiple areas of the outdoor dog runs in the facility that require maintenance. The metal tubing connections along the ground that secure the chin-link fence of the dog runs have areas that are completely rusted through. Additionally, the rusted connections create sharp edges and points along the bottom of the fence. Failure to maintain all surfaces in the housing facility inhibits proper sanitation and can create sharp edges/points that can cause injury to the animals. The licensee must ensure that all surfaces in the housing facility are constructed in a manner and made of materials that allow them to be readily cleaned and sanitized or removed or replaced when worn or soiled.   • Outdoor runs with sharp points

    The outdoor runs of the primary enclosures for approximately 32 dogs are not being properly maintained. Approximately 11 of the chain link fencing dividers have sharp points present along the concrete floor. Additionally, there are fence ties at differing heights in the runs that create sharp points. Failure to ensure that enclosures are free from sharp points or edges can expose the dogs to discomfort or injury. The facility must ensure that primary enclosures are constructed and maintained so that they have no sharp points or edges that could injure the dogs.
