* " Missy" a female Boxer (#153) has a dangling mass on the left side of her abdomen.

3/27/14 *********** * Three dogs were noted with hair coat maintenance problems. "Champi"- a white Maltese with no identification had matted hair on the hindquarters, " # 138- black Shih Tzu had matted hair on the face that dangled and pulled on the skin, also obscurring the dog's vision, and "Sage" (# 125) - a red male Pomeranian that had black organic material matted into the fur along the hind legs and feet.

3/31/14 ************ * A Chihuahua (#176) has long nails on the right front foot that bend to the side when the animal is standing.

"Champi", a white Maltese and "Joey" a male Pekingese did not have any individual identification on them. All dogs on the premise must have a unique individual identification tag on them to allow for identification. Licensee must ensure that all lost tags are replaced to allow for individual identification. ** To be corrected by: 4/7/14

An enclosure in the "Boxer" building (containing three dogs) had a portion of floor that was separated from the subflooring. This created a gap that could allow a foot to become caught and injure the dog. Licensee must replace or repair this flooring to keep the enclosure in good repair and prevent injury to the dogs within. * To be corrected by: 4/7/14

(2) The use of appropriate methods to prevent, control, diagnose, and treat diseases and injuries, and the availability of emergency, weekend, and holiday care; * A female Silky Terrier (#157) nursing two puppies (DOB 4-19-14) is very thin with easily palpable ribs and defined vertebrae. She had a build up of brown, gray-green material on her back teeth with gums that were swollen and when touched, a thick yellowish- white discharge occurred. She also has matted hair on the face and legs that are dangling and pulling on the skin. The appearance of vertebrae and ribs may be an indication of malnourishment, parasites or other illness. Swollen gums may make eating painful also. Hair that is matted and pulling on the skin can cause discomfort and pain to the animal. The licensee must remove the matted hair and have this dog examined by a veterinarian. The licensee must ensure that a effective hair coat maintenance program is adhered to at all times to prevent matting of the hair and that all animals receive adequate veterinary care with documentation (preferably in writing) of any treatments, prescriptions or medical programs prescribed.

(a) Structure; construction. Housing facilities for dogs and cats must be designed and constructed so that they are structurally sound. They must be kept in good repair, and they must protect the animals from injury, contain the animals securely, and restrict other animals from entering. * Four enclosures had holes (approximately 4 in. x 4 in.) that were previously used for self feeders on the top of the enclosure.Dogs were seen to push their head through the hole. These enclosures no longer protect the dogs from possible injury. Licensee must ensure that all housing facilities are maintained and protect the animals from injury and contain them securely. * To be corrected by: 6/17/14

(2) Maintenance and replacement of surfaces. All surfaces must be maintained on a regular basis. Surfaces of housing facilities--including houses, dens, and other furniture-type fixtures and objects within the facility--that cannot be readily cleaned and sanitized, must be replaced when worn or soiled. * The door frame on a wooden dog house with a male, Yorkshire Terrier was chewed all around the entrance edges. A water bowl (with three dogs) was excessively chewed down along one half of the bowl. These chewed surfaces are unable to be readily cleaned and sanitized. Licensee must ensure that all surfaces and objects within the facility are maintained and repaired/ replaced when worn, in disrepair or can no longer be readily cleaned and sanitized. * To be corrected by: 6/17/14

(2) The use of appropriate methods to prevent, control, diagnose, and treat diseases and injuries, and the availability of emergency, weekend, and holiday care; * "Maggie" #165, a Silky Terrier has knotted, matted hair pulling at the skin on her neck, legs and abdomen. … This matted hair is pulling on the skin and can cause pain.

* A Pomeranian #130 has matted hair around the anal region.

* A male Chihuhua #126 has dangling, matted hair on the hindquarters and long nails.

(2) The use of appropriate methods to prevent, control, diagnose, and treat diseases and injury. * A male boxer ("George" #121) was observed to have a deep, harsh, non-productive cough upon exertion. Licensee stated that he noticed the cough yesterday, however has not yet consulted a veterinarian about the cough nor scheduled any appointment for examination of the dog. Licensee must confer with a veterinarian about an appropriate diagnosis/examination/treatment plan. Licensee must maintain documentation of the consultation with any prescribed recommendations or treatments and have available upon request. ** To be corrected by: 1/15/15

Chihuahua (#190) has a torn left ear with the ear flap tip dangling. The edges of the wound are blackened. The ear flap has dried blood on it.

6/27/15 * Shih Tzu (# 187) was seen to periodically shake and rub her head. The right ear felt hot to the touch and had a black discharge in the ear canal.

* A white Pomeranian (# 84) has patches of matted hair along the belly and behind the ears. … The penis and prepuce looked red, inflamed and irritated.

* Silky Terrier (# 162) has long toenails that are starting to curve and/or have "hooks" at the end of them. These toenails do not allow the dog to stand in a normal postural manner and may cause injury if caught on the enclosure.

Three enclosures containing six dogs total, had areas of rusted metal siding that the dogs come in contact with. These rusted areas could harbor disease organisms as can not be cleaned and sanitized properly. Licensee must remove, repair or replace and maintain all surfaces to allow for required cleaning and sanitization to help eliminate disease hazards to the dogs. * To be corrected by: 7/24/15

At time of inspection the divider between two enclosures (2 Shih Tzu's with a Silky Terrier and 2 Boston Terriers) had come unattached from the main building. This created an opening which allowed the dogs access to each other, resulting in the death of Silky Terrier (# 110). Licensee must ensure that all housing facilities are constructed and maintained to be in good repair at all times to: protect the animals from injury, contain the dogs securely and prevent other animals from entering the enclosure. * To be corrected: 6/24/15 (Immediately)

There are two Chihuahua's housed together in an enclosure. Dog # 190 had a torn ear with the tip of the left ear flap dangling. The ear flap showed signs of dried blood and the edges of the wound had started to turn black. Licensee had not noticed at time of inspection but indicated that it may have been the result of a fight between the dogs. This type of injury is indicative of non compatibility between the animals to the point of injury. Licensee must ensure that all dogs housed together in a primary enclosure are compatible at all times and separate immediately if aggression issues are observed for the safety of the dogs. * To be corrected by: 6/24/15 (Immediately)
