In building B, there is a female, white Bichon (ear tattoo #79) that has a wound on the right side of her neck. The wound is red and parts of it are still raw. There is a dark brown material caked on the hair around the wound. The area of the neck containing the wound is slightly swollen. The right ear has some dried blood on the tip of it from touching the wound. Neck wounds such as this can cause pain and discomfort to the dog as well as lead to other health issues. This same dog also has a thick dark brown substance caking the surface of her check teeth making it difficult to see the teeth. Poor dental health can cause pain and discomfort to the dog as well as lead to other health problems. This dog must be evaluated by a licensed veterinarian for her neck wound and dental health. Licensee must obtain an accurate diagnosis and treatment plan from the veterinarian. This diagnosis and treatment plan must be documented and made available to the inspector upon request. Licensee must ensure that all dogs receive adequate veterinary care at all times. Correct by: May 04, 2015

There are two enclosures in building B and one enclosure in the far building that have an excessive buildup of dirt and grime on their walls. There are a total of three dogs housed in these enclosures. This buildup of dirt and grime can increase disease risks to the dogs. All used primary enclosures must be sanitized once every two weeks and more often if necessary to prevent the buildup of dirt, grime and disease hazards. Correct by: May 11, 2015

There are an estimated 25 dogs that have no official identification on them at time of inspection. Official identification is needed to ensure that each animal s identity can be known with certainty which helps to facilitate the inspection process. The applicant must ensure that all live dogs on the premise shall be identified by an official tag, or a distinctive and legible tattoo marking that is acceptable to and approved by the Administrator. If the licensee uses microchips as official identification, then a functioning microchip reader must be made available to the inspector at time of facility inspection. In addition, a variance from the regional office to use microchips must be maintained on file by the licensee.

Ten un-weaned puppies are being housed on square plastic flooring that will allow the feet and legs to fall through the openings in the flooring. The feet and legs of two puppies were observed falling through the openings of the enclosure flooring. The licensee had covered a portion of their enclosures with carpet squares. However, there is nothing to prevent these young puppies from moving to the uncovered portion of the enclosure where their feet and legs can fall through the openings in the flooring. The daily activity and mobility of the puppies are affected when there feet and legs fall through the openings. The potential for injury exists when the feet and legs pass through the openings in the flooring. All primary enclosures must have floors constructed in a manner that protects the puppy’s feet and legs from injury and does not allow the puppies' feet or legs to pass through any openings. No recorded date for annual heartworm testing for 125 dogs had been completed. Annual heartworm testing is important for the health of the animals. Heartworm testing is done to ensure that there are no active internal parasites. The applicant must ensure that all dogs are receiving vaccinations for contagious and/or deadly diseases of dogs (including rabies, parvovirus, and distemper) and sampling and treatment of parasites and other pests (including fleas, worms, coccidia, giardia, and heartworm) in accordance with a schedule approved by the attending veterinarian. This is a 1st Re-License Inspection for a Class A license.

The last documented visit to the kennel by the attending veterinarian was more than one year ago on 04-17-2025. Annual visits by your veterinarian to assess the premises where all dogs are housed are important for ensuring adequate husbandry and animal care standards are being maintained. Regularly scheduled visits, not less than every 12 months, by the attending veterinarian to the premises where animals are kept, are necessary to assess and ensure adequacy of animal and veterinary care as required under the Animal Welfare Act. The most recent documented annual veterinary exams by the attending veterinarian for all adult dogs in the kennel are dated 04-17-2025. Annual veterinary exams are important to evaluate the health of all breeding stock and to detect diseases or conditions that may impact the health and well-being of animals. A complete veterinary examination, from head to tail, of each adult dog by the attending veterinarian is required not less than once every 12 months.
