“The written Program of Veterinary CareA written plan, signed by a vet, for the animals' medical care. form states that all adult dogs will be given '1 dose annually' for vaccinations against parvovirus, distemper, Leptospirosis and Bordetella. The last documented record of adult dogs receiving their vaccinations was dated 7-23-24. The PVC also states that 10% of the dogs will have fecal exams done on a yearly basis. There are no records to indicate that 10% of the dogs had fecal examinations done within the past 12 months. ...”
