Health Certificates The licensee is using heath certificates designed and for use on animals sold and transported in the State of Wisconsin only. However, APHIS Form 7006's show multiple sales in which the animals are being transported across the state lines. Health certificates must be correctly filled out, and followed as an assurance that the animal(s) are healthy and that travel is acceptable. All federal, state, and local laws, regulations, and guidelines involving the transportation of animals must be strictly followed. Correct by 6/20/2014

The first dog was a black male Shipoo puppy born 10/4/17. The puppy had been given lactated ringer fluids by the licensee. After receiving fluids the puppy lost its hair in the area surrounding where the needle had been inserted under the skin. At least three scabs were observed in this area. In addition this puppy's left eye was clouded over. The licensee had observed the hair loss on the back, but had not noticed the eye prior to inspection.

An adult female West Highland Terrier (microchip #004-368-822) had a puncture wound on the right side of the back of its neck. The licensee had not observed this prior to inspection.

Two dogs had veterinary care issues that had not been addressed by the Attending Veterinarian. … The Attending Veterinarian had not been contacted for either dog.

The first dog was an approximately 3 year old female poodle (microchip #900217000187655) that had wounds covering its body. In addition, the dog was limping on its left rear leg.

Another dog, an approximately 7 year old female poodle (microchip #900111881600916), had an open wound on its abdomen. I originally observed the dog sitting in the corner with a black substance on the upper part of its left rear leg. Upon closer observation, I observed an open wound on its abdomen and a reddish/pinkish liquid oozing/dripping off of the dog. In addition, the dog appeared to have a very difficult time moving around and appeared to be either extremely uncomfortable or in pain. The dog seemed to be lethargic.

At the time of inspection there was no written exercise plan available for review despite dogs over 12 weeks of age being present at the facility. Exercise for dogs is critical for healthy development and psychological well-being. The licensee must develop, document, and follow an appropriate plan to provide dogs with the opportunity for exercise. In addition, the plan must be approved by the attending veterinarian. The plan must include written standard procedures to be followed in providing the opportunity for exercise. Correct starting on 8/22/25.

During the inspection, I observed a Golden Retreiver trying to stick its head under the fence to lick up the water that pooled up under the watering nipple in the enclosure adjacent to it's enclosure. I asked the licensee to test the watering nipple and no water was coming out when she pushed down on it. She provided the three dogs in that enclosure with a bowl of water and I observed them to be drinking out of that bowl non-stop for approximately 30-45 seconds. Dogs that don't have access to water can result in the dog becoming sick and potentially lead to death. It is important for all dogs to have continuous access to potable water to maintain good health and hydration. Correct by making sure all watering nipples are in working order and/or providing all dogs continuous access to potable water. Correct starting on 7/22/25.

A female maltese (microchip #900164001625419) had hair loss on the top of its head along with an open wound and dried up blood surrounding the wound. The licensee had observed the hair loss prior to inspection, but had not observed the open wound prior to inspection and was not treating the dog.

A tan female Doodle (microchip #900217000187652) was observed to be limping on its left rear leg. There were no obvious wounds or injuries to the leg during examination. The licensee had not observed this dog to be limping prior to inspection.
