- Facility unavailable for inspection
On July 31, 2014 at approximately 1245 this facility was not able to be inspected for compliance with the Animal Welfare Act regulations and standards to ensure the welfare of the animals. A responsible adult must be made available to accompany the inspector on an animal welfare inspection. Correct for all future unannonced inspections.

- Male Yorkie with loose front teeth
Male Yorkie #108 has signs consistent with advanced dental disease. The gums of all front incisors and canine teeth are receded and several front teeth are loose.
- Male Maltese with reddened back lesion
**An approximately one year old Maltese male (no ID) was found with a skin problem. There is an area if skin that is reddened (approx. 2 inches in diameter) and a scab (approx. 0.5 inches in diameter) near midline of the back just below the shoulder blades. The hair in that area is also discolored to an orange tint. The licensee had this dog examined on 5-13-14 and the veterinarian dispensed shampoo for use.

- Female Shih Tzu neck lesions (Repeat)
A Female Shih Tzu (#148) had lesions under her neck. The underside of this dog's neck had an area approximately 3 inches long and 1.5 inches wide that had an area of reddened skin, lesions and scabs. After this dog was pointed out and caught, the facility representative said they had treated this dog for neck problems, but was not under any current treatment and didn't have any documentation of prior treatment.
- Unlabeled medication transfer bottles (Repeat)
The facility as had four transfer bottles that were not properly labeled. The facility was using old medicine bottles for transfer bottles, with no product name or expiration date of what was in the bottles. Some of the other bottles did had a product name wrote on the bottle but no expiration date. These medications were stored with other medications that were currently being used, and therefore ready to use.
- Yorkie and male Maltese care addressed (Repeat)
The veterinary care for Yorkie ( 108) and male Maltese ( no ID ) from the 10/28/2014 report have been addressed.
