  • Two Maltese females without official ID

    Two maltese females, 16 weeks of age (born 1/28/15), did not have official identification such as a tag, tattoo or microchip. Facility representative said that she ran out of microchips and has to order some from her supplier. Failure to properly identify all dogs on premises makes tracking of dogs difficult. Licensee must ensure that all dogs are properly identified. Cage cards are acceptable when puppies are less than 16 weeks of age and maintained as a litter in one enclosure. Dogs 16 weeks and older must have official identification. Correct by June 15, 2015

Licensee must keep and maintain records of the names and dates of all vaccines and treatments given to adult dogs. Correct by May 26, 2022.

    The cage cards on the primary enclosures for 20 puppies did not include a unique number identifying each puppy. Failure to identify puppies with a unique identification number makes it difficult to keep track of puppies born, raised, or died. Licensee must keep and maintain records on APHIS form 7005, or on cage cards placed on the animals' primary enclosure for puppies under 16 weeks of age which include a unique identification number for each puppy. Correct by October 1, 2023.

  • Adult dogs missing vaccine dates

    There were no dates recorded for when the annual parvovirus and distemper vaccines were last given since 2022 for the adult dogs. Failure to record the dates of all vaccines and treatments administered to adult dogs makes it difficult to track if vaccines and treatments are being done as prescribed in the written program of veterinary care (APHIS form 7002A). Licensee must record the dates of all vaccines and treatments given to the adult dogs to ensure adequate veterinary care is provided. Correct by October 1, 2023.
