One plastic bottle containing Pyrantel wormer has no written directions on how this drug is to be given and at what rate it is to be administered. In addition, a recorded expiration date was missing from the label. This drug is intermingled with other products that are currently being used at the facility and is therefore ready for use. Medications must be labeled with appropriate and accurate information in order to ensure that the contents and dosage rates of the container can be known with certainty and not used past their expiration date. The effectiveness of the dosage level and expected results could be drastically changed or may not work as anticipated if the expiration date is unknown. The licensee must ensure that all medications are labeled with dosing instructions specific for the species of animals. In addition, an expiration date must be recorded on the label. No inventory information was available on 18 unweaned puppies. Inventory information is needed to ensure the individual identity can be known with certainty and to facilitate the ability to trace all puppies. The licensee must make, keep and maintain complete and accurate inventory records that fully and correctly disclose information on all animals including any offspring born at the facility. Correct by 7/28/15.

An estimated two by three inch section of missing fiberglass was observed on the interior wall of one enclosure. In addition, the wallboard had a tear extending in the opposite directions from the missing section of fiberglass. Two dogs are in contact with the affected area located in sheltered housing. Jagged, sharp edges could cause potential injury. The licensee must ensure all surfaces are free of jagged, sharp edges. Correct by 7/28/15.

No copy of the program of exercise for 42 dogs was made available at time of facility inspection. The program of exercise must be developed to ensure the dogs are receiving adequate exercise. The licensee must develop, document and follow an appropriate plan of exercise, the plan must be approved by the attending veterinarian and be made available to APHIS upon request. The USDA Program of Veterinary Care form 7002 A is not being followed for 44 dogs. Under section B “Parasite Control Program” it states under Ectoparasites that Ivermec will be given twice a year as a pour on to the adult dogs. Review of the medical records show that no Ivermec has been given to the dogs. The program of veterinarian care establishes standards of care for the animals. Medical records provide information that the program of veterinary care is being followed and when contact with the attending veterinarian is needed. It shows what treatments and treatment dates have been provided to the dogs. The licensee stated that the dogs are receiving Ivermec twice a year but failed to have a record of the treatment and treatment dates in their medical records. The licensee must keep a record of all treatments administered and the dates of administration. These records must be made available to APHIS officials upon request. Correct by 10/8/24.
