The effectiveness of the dosage level and expected results could be drastically changed or may not work as anticipated if the expiration date is unknown. The licensee must ensure that all medications are labeled with dosing instructions specific for the species of animals. In addition, an expiration date must be recorded on the label. In addition, the wallboard had a tear extending in the opposite directions from the missing section of fiberglass. Two dogs are in contact with the affected area located in sheltered housing. Jagged, sharp edges could cause potential injury. The licensee must ensure all surfaces are free of jagged, sharp edges. Review of the medical records show that no Ivermec has been given to the dogs. The program of veterinarian care establishes standards of care for the animals. Medical records provide information that the program of veterinary care is being followed and when contact with the attending veterinarian is needed. It shows what treatments and treatment dates have been provided to the dogs. The licensee stated that the dogs are receiving Ivermec twice a year but failed to have a record of the treatment and treatment dates in their medical records. The licensee must keep a record of all treatments administered and the dates of administration. These records must be made available to APHIS officials upon request.
