- Yorkshire Terrier salivating, loose tooth
The kennel facility consists of five sheltered buildings. In the eastern side of the fourth building there is one nine year old male Yorkshire terrier, chip# 071835025, which needs to be evaluated by the attending veterinarian. During the inspection the animal was observed excessively salivating and upon examination had a tooth on the bottom right side which was loose and the dealer was able to move it about. The dealer must maintain written documentation of the evaluation and all treatment. Affects one adult Correct by May 22, 2014

In building #1 there is an adult male Shih Tzu who is ten (10) years old, chip #107283045, named Rocko. Both of the animals eyes are cloudy with green matter on the exterior.

One is a ten (10) year old Yorkshire terrier, chip #0722888590, named Jenny who was observed non-weight bearing on her left front paw.
- Male Maltese black buildup on canines (Repeat)
In the same building there was an adult male Maltese, chip #032770312. … The animal had black buildup on both canine teeth with dark red gum line.
- Female Maltese green buildup on teeth (Repeat)
One is a ten (10) year old female Maltese, chip #107285065, which had a buildup of green matter on both canine and cheek teeth.
- Male Yorkshire terrier four teeth, green buildup (Repeat)
The other animal is a male adult Yorkshire terrier, chip # 107258628, which had only four teeth, two upper and two lower. All four of the teeth had a buildup of dark green matter on the tooth surfaces with white matter on the gum line.
- Five-month Maltese green eyes, draining shoulder (Repeat)
One is a five (5) month old female Maltese puppy, chip #040380814. … During the inspection the animal had green matter on both eyes and a hair loss which was a red draining area approximately the size of a quarter on its right shoulder.

The other animal in the whelping building is one adult female two (2) year old King Charles Cavalier, chip # 003562514 who has a large hair loss area on her right hip area. … The animals hair is falling out in clumps.
