  • No contingency plan

    The facility does not have a contingency plan in place. All licensees must have a contingency plan in place to provide humane care, housing and transportation for the animals in case of an emergency/disaster. The facility must develop a contingency plan to address any and all emergencies/disasters that the facility might encounter on how to provide for humane care of the animals at the facility if such would occur. To be corrected by: 09-19-2022.

  • No veterinary care program

    The facility does not have a current program of veterinary care for dogs. The facility must develop, documented in writing and signed by the attending veterinarian. The program must include all preventive vaccinations to include rabies, all parasitic preventions to include testing for internal parasites. A yearly hands on head to tail exam by a veterinarian, to include a site visit not less than once every 12 months. Also include methods to ensure healthy hair coats, healthy and clean eyes, ears, skin and teeth. All licensees with regulated dogs are required to have a program of veterinary care for dogs to provide for the health and well-being of the dogs. To be corrected by: 09-25-2022.

    ** A six year old date of birth 12-21-17, female black and silver Poodle, micro-chip 2194966. Had a buildup of dark matter on the teeth and red gums.

  • Male Yorkie dental buildup

    ** A five year old, date of birth 12-02-18, male, black and Tan Yorkie, micro-chip 2874016. Had a buildup of dark matter on the teeth and red gums.

  • Missing medical records for dogs

    The licensee did not have medical records for the dogs. All licensee's must have medical records for the dogs. To include identity of the animal, breed, sex and age.
