The sales were documented in writing in another format. Failure to utilize required APHIS forms which fully and correctly disclose the number of animals disposed of can result in not having complete and accurate information to track animals being used in regulated activities to ensure their humane care and use. The licensee shall make, keep, and maintain records of disposition of dogs that fully and correctly discloses the required information utilizing the Record of Disposition of Dogs and Cats (APHIS Form 7006).
