Some of the sale forms (APHIS form 7006) did not have the official puppy identification number recorded. The puppy identification number must be recorded on APHIS form 7006 to help identify animals, document changes in ownership, and keep track of animal shipments. Licensee must ensure that dogs and puppies are officially identified. Correct by August 16, 2014

Since the previous inspection report of August 12, 2014, majority of the puppies sold were listed on a bill of sale instead of APHIS form 7006. Each dealer is required to use the Record of Disposition of Dogs and Cats (APHIS Form 7006) to make, keep, and maintain the information required by paragraph (a)(1) of this section, unless the dealer has obtained a variance to use a form other than APHIS form 7006. Licensee must complete and maintain a copy of APHIS form 7006 for all dogs and puppies sold to comply with the regulations, document changes in ownership, and to keep track of animals. Correct from this day forward.

The written program of veterinary care is incomplete. There is no written guidance from the attending veterinarian on the name of drug, concentration, instructions for use, dose, frequency, and route of administration for ivermectin, marbenflaxen, and fish mox. The licensee should ensure that he has written guidance from the attending veterinarian on the name of medication, dosage, concentration, instructions for use, and route of administration on all medications. This is to ensure the attending veterinarian is aware of all treatments taking place so that animals are given appropriate care. Correct by March 15, 2017.

Storage. There are bags of bedding stored on the floor and open bags of bedding in the kennel. Open supplies of food and bedding must be stored in leakproof containers with lids. Bags of food and bedding must be stored off the floor and away from walls to allow cleaning underneath and around supplies. Correct by storing open bedding in closed containers and bags of bedding off the floor to protect supplies from contamination and vermin infestation. Correct by February 25, 2019.

Wardens noted 3 dogs in the kennel facility did not have current rabies vaccinations given within four weeks after the date the dogs attained 12 weeks of age.

This Warden viewed nine(9) primary enclosures that did not meet the minimum size requirements as required by this section.

There were no samples taken to monitor for internal parasites for the adult dogs in the last 12 months. The written program of veterinary care states that composite fecal samples on adult dogs will be conducted at least annually. Failure to conduct annual sampling can effect the preventative program for internal parasites established for the adult dogs. Licensee must follow the written program of veterinary care on file for sampling for internal parasites. Correct by November 1, 2023.

This warden observed 8 adult dogs that did not have current rabies vaccinations.

A veterinary examination was ordered on a dog under section 402(a) based on warden’s observations of the condition of dogs during inspection. Kennel is in compliance with section 402(d) because veterinary examination was completed within the 72-hour time frame.
