    Some of the sale forms (APHIS form 7006) did not have the official puppy identification number recorded. The puppy identification number must be recorded on APHIS form 7006 to help identify animals, document changes in ownership, and keep track of animal shipments. Licensee must ensure that dogs and puppies are officially identified. Correct by August 16, 2014

  • Puppies sold without APHIS form 7006

    Since the previous inspection report of August 12, 2014, majority of the puppies sold were listed on a bill of sale instead of APHIS form 7006. Each dealer is required to use the Record of Disposition of Dogs and Cats (APHIS Form 7006) to make, keep, and maintain the information required by paragraph (a)(1) of this section, unless the dealer has obtained a variance to use a form other than APHIS form 7006. Licensee must complete and maintain a copy of APHIS form 7006 for all dogs and puppies sold to comply with the regulations, document changes in ownership, and to keep track of animals. Correct from this day forward.

The licensee should ensure that he has written guidance from the attending veterinarian on the name of medication, dosage, concentration, instructions for use, and route of administration on all medications. This is to ensure the attending veterinarian is aware of all treatments taking place so that animals are given appropriate care. Correct by March 15, 2017.

    Storage. There are bags of bedding stored on the floor and open bags of bedding in the kennel. Open supplies of food and bedding must be stored in leakproof containers with lids. Bags of food and bedding must be stored off the floor and away from walls to allow cleaning underneath and around supplies. Correct by storing open bedding in closed containers and bags of bedding off the floor to protect supplies from contamination and vermin infestation. Correct by February 25, 2019.
