Four Maltese dogs that were >16 weeks old did not have individual identification on them. Four weaned Maltese puppies that were from three separate litters were housed together in an enclosure. None of the puppies had proper individual identification. Appropriate identification of dogs is necessary to meet recordkeeping requirements for inventory, disposition, transport, and healthcare documentation. The licensee must individually identify puppies that are from different litters and housed together in the same enclosure. Acceptable forms of identification include a microchip, a collar and tag, or a plastic-type collar with the required information legibly written. TO BE CORRECTED BY DECEMBER 21, 2014.

At time of inspection an adult female Maltese "Lou Lou" (m/c# 074550633) was exhibiting a thin body condition. Her vertebra were visible and easily palpable.

At time of inspection an adult female Maltese "Lou Lou" (m/c# 074550633) was exhibiting a thin body condition. … Upon further hands on inspection of the dog it was noted that the teeth had a significant build up of tartar and the gum line was red and swollen.

There was a clear bag of pills that had no label. … However, the licensee did not know the name of the medication and did not have directions on how to administer.

A bottle of Cephalexin Capsules, with an expiration date of March 2015 and a bottle of Cystorelin with an expiration date of 02-2016 were located with other medications ready for use.

The licensee was not able to produce an up-to-date APHIS form 7005 (Dogs on Hand) during the inspection. Thirty-eight dogs were missing from the form. Properly completed acquisition records are necessary for identification of dogs to facilitate record-keeping requirements for inventory, disposition, transport, and healthcare. The licensee must update the Record of Acquisition and maintain the record on a regular basis so that it always has accurate information regarding acquisition, identification, and disposition for all dogs. TO BE CORRECTED BY JUNE 1, 2024.

The licensee is using cloth towels as additional wind breaks over the metal doggie doors in the sheltered building. The towels have excess dirt and grime on them and the licensee stated that she is not routinely cleaning or replacing these towels. Dirt and grime harbor organisms that can cause disease in dogs. The licensee must ensure that surfaces that come in contact with the dogs are cleaned and sanitized at least every 2 weeks or more often as necessary to satisfy generally accepted husbandry standards and practices.
