There are three bottles of expired injectable medications in the refrigerator of the whelping building: Combi-Pen-48 (2/2013), Bactracillin G Benzathine (5/2015), and Cal-Phos Sol (Nov 2010). The usage of expired medical materials such as drugs, fluids, or sutures on regulated animals is not considered to be an acceptable veterinary practice and does not constitute adequate veterinary care. The effectiveness of the dosage level and expected results when expired drugs are administrated to animals could be drastically changed or may not work as anticipated. The applicant must ensure that all medications used are not expired and labeled properly in accordance with standard veterinary practices. To be corrected by 23 June 2015.

The feet and legs on one Shiba Inu mix puppy were seen to pass through the openings of the floor of the enclosure. Dogs whose feet are able to pass through the openings in the enclosure floor are at increased risk for injury. The licensee must ensure that all primary enclosures have floors that are constructed in a manner that protects the dogs' and cats' feet and legs from injury, and that, if of mesh or slatted construction, do not allow the dogs' and cats' feet to pass through any openings in the floor. To be corrected from this point forward.

- The metal lining in three egress doors in the second building inspected had a rough, deteriorated surface at the base. - One plastic floor board in the second building inspected had rough and torn edges from being chewed. The chewed edges are deeply pitted and dirty. - Three enclosures in the third building inspected had support bars along the base of the outdoor portions with peeling and deteriorated rubber/truck bed liner leaving a rough surface and exposed rough metal. - Two enclosures in the fourth building inspected had support bars along the base of the outdoor portions with a deteriorated painted metal surface leaving a rough, rusted surface. Surfaces that are rough, rusted, and deteriorating are not able to be adequately cleaned and sanitized and may play a role in disease transmission. The licensee must ensure that surfaces of housing facilities--including houses, dens, and other furniture-type fixtures and objects within the facility--must be constructed in a manner and made of materials that allow them to be readily cleaned and sanitized, or removed or replaced when worn or soiled. The plastic floor board was corrected during the inspection. The rest to be corrected by 3 September 2016.

There are seven enclosures with bent wires or flooring surfaces turned in to the enclosure space exposing metal points. Sharp metal points can cause injury to dogs. The licensee must ensure that primary enclosures be constructed and maintained so that they have no sharp points or edges that could injure the dogs. To be corrected by 5 March 2020.

There are at least four water receptacles that have black debris coating the interior surface and floating in the water. Failure to keep food and water receptacles clean and sanitized can play a role in disease transmission. The licensee must ensure that used food and water receptacles for dogs and cats must be sanitized at least one every 2 weeks using one of the methods prescribed in paragraph (b)(3) of this section, and more often if necessary to prevent an accumulation of dirt, debris, food waste, excreta, and other disease hazards. To be corrected by 5 March 2019.

The medical records for 154 dogs are incomplete. The physical exams for these dogs completed within the last 12 months are not accounted for in the medical records. The Attending Veterinarian verified that all physical exams were completed but not documented in the medical records for the dogs. Complete medical records allow the attending veterinarian, the licensee and all animal caretakers to ensure that the medical needs of the dogs are accounted for and addressed. The licensee must keep copies of medical records for dogs and make the records available for APHIS inspection. These records must include the dates and findings/results of all screening, routine, or other required or recommended tests or examinations. To be corrected by 8 April 2024.
