Lack of official USDA identification does not allow for the proper identification or trace backs of the animals. Licensee must identify all dogs over 16 weeks of age on the premise with official USDA tags.

Thirty-six weaned puppies under sixteen weeks of age do not have individual identification numbers listed on their cage cards. Lack of identification numbers does not allow for the proper identification or trace backs of the animals. Licensee must identify all puppies with official identification.

- Mixed breed eye discharge (Direct)
A female, mixed breed dog identified with USDA tag #20 was observed to have a large amount of dried and thick, sticky discharge on and around both eyes. The licensee stated that the dog's eyes have been this way for approximately six weeks and that he has been treating them with a spray used for pink eye. The medication is not listed as a treatment for eyes in the Program of Veterinary Care and the Attending Veterinarian (AV) has not evaluated the dog's eyes. Large amounts of discharge in and around the eyes is abnormal and can be a symptom of serious and painful health conditions. Using an unapproved medication in the eyes, without the guidance of the AV, may do nothing to correct the underlying condition and may worsen the problem. The dog's eyes must be evaluated by the AV and the prescribed treatment must be followed.

The licensee did not use APHIS Forms 7005 and 7006 for three adult dogs that he received on 7/8/25 and have since left. Not using APHIS forms 7005's and 7006's for all dogs that move in and out of the kennel does not allow for proper tracking of these dogs by APHIS officials. The licensee must use APHIS Forms 7005 and 7006 for all dogs moving through the kennel from here on out. Correct starting on 9/29/25.
