    The licensee moved to a new address and did not notify Animal Care of his change of address. The licensee is currently operating his kennel from an unapproved site. Conducting regulated activity from an unapproved site does not allow APHIS Officials to know the location of the animals and does ensure optimal animal welfare and well-being. The licensee shall notify Animal Care no fewer than 90 days and obtain a new license before any change of address. Correct by:

    A black female poodle (#01) requires dental care. In April, at the time of the dog's yearly physical, the Attending Veterinarian indicated that the dog had a moderate buildup of tartar on her teeth and needed to have her teeth cleaned. The licensee has not yet worked to get the dog's teeth cleaned and a thick coating of dark tartar remains on the dog's teeth. Tartar buildup on the teeth can cause a painful mouth and infections of the gums and throughout the body if not removed. The licensee must provide appropriate dental care to the dog. Correct by June 7, 2023.
